Before we can give you any accurate answers we need a little more info.
What type of thermometers are you using?
There is no set distance for the heat bulb. That needs to be placed at the distance that gives you a temp of 110* to 120* on the basking surface.
Distance for UVB depends on what type of UVB you're using. Please provide us with the brand, rating, and bulb style that you've chosen.

For the safety of your dragon the basking light should be placed far enough away that you dragon cannot accidentally touch it and burn himself.  I would say a minimum of 6-7" depending on how far your dragon can jump.  If you can't reach the necessary temperature at that distance you'll need a higher watt bulb or in very large enclosures sometimes a second incandescent bulb.  If the next step up (eg 100W to 150W) is too hot a dimmer can help.  A standard household lightbulb is all you need for heat.  The best UVB on the market right now is the reptisun 10.0 and you can get them cheaply at www.petmountain.com.  Place the UVB 6-10 inches away.

The analog ( stick on) thermometers are pretty much useless for a dragon.. You'll want to find either a digital with probe thermometer or a temp gun.  I found my temp gun at a auto parts store for fairly cheap.
Just check all your local parts stores.. They'll know what you are asking for.

Naaaaaaaaaaa I don't see a need for that.  You can of course if you want to, but it really isn't needed.

Hope this helps !

Only really necessary if you don't have central heating or A/C, or if you live somewhere that's especially dry or humid.
